Heart failure with reduced ejection fraction (HFrEF) continues to be associated with substantial morbidity and mortality despite significant advances in therapeutic options. In the era of guideline-directed medical therapy (GDMT), a growing body of evidence has highlighted the importance of accurate diagnosis, early patient identification, and timely initiation of comprehensive treatment strategies. The scientific discussion emphasized innovative approaches to HFrEF management, with particular focus on recent analyses that refined diagnostic pathways, improved patient selection, and expanded real-world understanding of clinical outcomes. The aim was to examine how contemporary evidence could be translated into more effective and individualized treatment decisions. The findings were presented at Heart Failure 2026, organized by the European Society of Cardiology, held in Barcelona, Spain, from 9–12 May 2026.
Recent clinical analyses, subgroup evaluations, and practical experience from HFrEF management were reviewed. The discussion explored current diagnostic criteria, biomarkers, imaging-based assessment, and strategies for identifying appropriate candidates for evidence-based therapies. Attention was also given to the implementation of GDMT, including optimization of foundational therapies and the challenges encountered in routine practice.
The reviewed evidence indicated that early and precise diagnosis enabled more timely therapeutic intervention and improved risk stratification. Better patient selection was shown to enhance treatment responsiveness and support more personalized care. Clinical experience suggested that rapid initiation and up-titration of GDMT were associated with improved symptom control, fewer hospitalizations, and lower mortality. Emerging analyses also demonstrated that treatment benefits were consistent across a broad spectrum of patients, including older adults and those with multiple comorbidities.
Innovative diagnostic tools and better patient selection strategies were shown to strengthen the management of HFrEF. When integrated with optimized GDMT, these approaches were associated with improved clinical outcomes and offered a more structured and effective pathway for long-term heart failure care.
