Study Background:

Despite established guidelines, hypertension remains poorly controlled globally, particularly in primary care. Barriers to guideline implementation include structural, behavioral, and communication challenges across healthcare systems. This session outlined practical strategies to transform everyday hypertension management using insights from clinicians across Europe.

Methodology & Framework:

  • Expert contributions were crowdsourced across multiple European countries.
  • Key practice change priorities were organized into five actionable domains (ABCDE):
    • A – Accuracy & Assessment: Emphasized out-of-office monitoring (ABPM/HBPM) and the need for wearable validation protocols.
    • B – Behaviour & Bad Organs: Advocated opportunistic BP measurement by all healthcare professionals and proactive organ damage screening.
    • C – Communication: Recommended initiating treatment with dual-agent single-pill combinations (SPC) to improve adherence and overcome clinical inertia.
    • D – Daily Routine & Diet: Prescriptive dietary targets supported by a multidisciplinary team approach.
    • E – Exercise: Structured exercise prescriptions with digital self-tracking by patients.

  • Communication should be team-based and patient-centered, addressing quality-of-life impact rather than medication adherence alone.
  • Integration of validated technology, simplified regimens (SPCs), and shared decision-making are essential for effective hypertension management.

Transforming hypertension care requires immediate, actionable change across five interconnected domains. Clinicians should empower patients, use validated monitoring technology, simplify treatment, and deliver care through coordinated, team-based models to achieve sustainable BP control and improve cardiovascular outcomes.
