Therapeutic Context:
Renal denervation (RDN) is emerging as a third pillar of hypertension management, alongside lifestyle modification and pharmacotherapy, for selected patients with uncontrolled or resistant hypertension. The clinical question is whether RDN provides benefits beyond BP reduction and whether a single intervention is sufficient for BP control.
Evidence from Clinical Studies:
- RDN achieves BP reductions comparable to intensified pharmacotherapy across office, home, and 24-hour ambulatory measurements.
- Systematic review and meta-analysis: RDN vs addition of a single antihypertensive drug shows similar 24-hour BP reduction and hypertension control rates (~35%), supporting RDN as an effective alternative to treatment intensification.
Potential Benefits Beyond Blood Pressure Reduction:
- Investigated in heart failure, atrial fibrillation, ventricular arrhythmias, CKD, sleep apnea, and type 2 diabetes through sympathetic modulation.
- RDN improves hepatic steatosis in hypertensive patients with metabolic syndrome.
- Preclinical studies of hepatic artery denervation in high-fat diet models demonstrate improved glucose tolerance, enhanced insulin secretion, reduced norepinephrine levels, favorable lipid profile changes, and reduced hepatic steatosis.
Clinical Implications and Future Directions:
- Multi-organ denervation (renal + hepatic) represents a novel strategy to achieve broader sympathetic nervous system modulation.
- Targeting richly innervated peri-vascular beds beyond renal arteries may enhance treatment effectiveness.
- Early preclinical and pilot clinical studies support this approach as a promising adjunctive therapy in complex hypertensive and cardiometabolic patients.
RDN provides clinically meaningful BP reductions comparable to pharmacotherapy and may offer additional metabolic benefits. Multi-organ denervation strategies have the potential to expand the therapeutic scope of interventional hypertension management beyond BP control alone.
